Application Process?
EAGLE Academy Application Process
- Complete and submit the online Student Application.
- Use this Flipgrid link (https://flipgrid.com/
devine3903 ) to film a quick video of yourself. Tell us your name, where you are from, something unique about you, and why you want to go to college.” - Once you have submitted an application, we will email you a link to upload the forms listed below:
- Submit a completed Personal Support Inventory (one from parent/guardian and one from a teacher/employment supervisor) (form is located on the Eagle Academy website).
- Submit three letters of recommendation from non-relatives who have known you for at least six months. At least one letter should be from a teacher (form on website).
- Submit Emergency Medical Treatment form (form on website).
- Submit proof and Acknowledgement of Guardianship, if needed (form on website).
- Submit Release and Exchange of Information form (form on website).
- Submit an official copy of high school transcripts and any other post-secondary education institutions that you may have attended.
- Submit Application Certification and Georgia Southern University Honor Code (form on website).
- Submit Lawful Presence Form (form on website).
- Submit a copy of your IEP from your last year in high school.
- Submit a copy of your Summary of Performance documents from high school.
- Submit a copy of your Individual Plan of Support from Vocational Rehabilitation. If you don’t have this yet, submit documentation (e.g., letter) stating what your status with Vocational Rehabilitation is.
- Submit a copy of your most recent psychological-educational evaluation.
- This evaluation needs to include both an achievement and adaptive assessment that are less than three years old*, AND
- An IQ test that is less than three years old, or two IQ assessments that have commensurate scores regardless of when they were given.
- *If you have been out of school for more than three years, please submit the most recent of the above named documents.
- Submit a copy of your Functional Behavior Assessment, if applicable.
- Submit a copy of your Immunization Record
- Complete a daily living skills inventory, if requested by the EAGLE Academy Director.
- After we have received an reviewed all of the above materials, we will contact you about the next steps in the admission process.

Eligibility?
We are excited about your interest in EAGLE Academy at Georgia Southern University! We must have a complete application packet before the admission screening team will review your application for potential enrollment. We carefully review and assess all information in order to make certain that a candidate will be successful at EAGLE Academy.
Information about the students we serve:
- We are a comprehensive transition program designed to serve students with an intellectual disability or developmental disability. Although applicants may have other disability diagnoses (e.g., autism, functional delay), our emphasis is on students who also have cognitive impairments.
- We serve students with disabilities who have successfully completed high school, and have received a high school diploma (regardless of the type of diploma). Students would have received or been eligible for services under IDEA.
- We enroll students who express a strong personal desire to attend college.
- We prioritize applicants who:
- are excited to become better prepared for the world of work and a future career.
- demonstrate both the need and a desire for greater independence.
- can learn to navigate the college campus on their own.
- can communicate reliably with others and exhibit socially responsible behavior on campus.
- have satisfactory record of school attendance from their previous school.
All enrolled students must:
- Have health insurance (Medicaid, Medicare, private).
- Have his/her personal cell phone upon entering the program, so EAGLE Academy staff can maintain contact.

Cost and Tuition Assistance?
Costs
EAGLE Academy students pay the same tuition, housing, and other fees that all Georgia Southern University students pay. Tuition and fee information may be found on the Bursar’s website.
EAGLE Academy students will reside on campus and each student will require to purchase a Dining plan.
EAGLE Academy fees are $5,000.00 per semester.
The University reserves the right to make changes in its fees, which are regulated by the GA Board of Regents, at the beginning of any semester and without previous notice.
Tuition assistance
As a designated Comprehensive Transition Plan (CTP), Federal Financial Assistance is available to students wishing to attend in the form of Pell Grants and Federal work study. Families must complete a FAFSA to determine eligibility. Eligibility is based on need. Students who are 23 years of age and older, do not need to consider their parent’s income. Some funding support may be provided through the Georgia Vocational Rehabilitation Agency (GVRA). EAGLE Academy students must have an approved case with GVRA to receive this funding.
